Medical Information and Consent Form for under 18s
As Freax is a licensed adventure activity provide (with the Adventure Activities Licensing Authority) we can provide activities for under 18's without their parents needing to be present. Freax consent forms provide the instructors with all the necessary information needed should there be an emergency and provides consent to participate in the activities from those legally responsible.
